Registering with us

Before registering with us, check you are in our catchment area. Find out which GPs you can register with on the NHS website.

New patients registering at the surgery will only be eligible if they live within a 3 mile travelling distance, excluding postcodes CH1 5**, CH1 6**, CH4 7** and CH4 8**. The three mile travelling limit will be determined by using Google Maps. If the address is in Wales, we are not able to accept the registration due to different healthcare, IT systems, and administrative arrangements in operation there.

Existing patients that move house more than a quarter of a mile beyond our 3 mile limit (or into one of these specific postcodes) will be asked to re-register at a new surgery.

How to Register

1. Complete and submit the form by clicking HERE then move on to step 2.

2. Complete a Medical Questionnaire, which can be downloaded HERE. (Paper copy of the form may be collected from the Practice)

3. Provide two separate forms of identification; photographic evidence of ID and proof of address as listed below

If using the the editable version of the GMS1 form, please save and email a copy to us, along with the Medical Questionnaire and your two forms of ID to boughton.registrations@nhs.net (please do not send any other enquiries to this e-mail address).

You may also bring both completed forms to the Practice, along with your ID and either post in the secure locked box in the conservatory, or hand to a member of the Admin Team.

Medical treatment is available from the date of registration. Please contact the Admin Team for further information.

NOTE:  Newborn Baby Registrations must be completed on paper forms obtained from the Surgery.

Below is a list of acceptable identification, please ensure one is photographic and one proving address:

  • Passport
  • Driving license
  • Blue Badge pass
  • Local authority tax bill/council tax bill for current year
  • Mortgage statement – less than 12 months old
  • Utility bill – less than 3 months old
  • Bank, building society or credit card bill – less than 3 months old
  • Payslip
  • Letter from benefits Agency/Benefits book/signing on card
  • Papers from the home office

Electronic Transfer of Records

If you have previously registered with a different GP in England, upon registering at this practice your electronic health record will, where possible, be transferred automatically from your previous practice through the use of an NHS system called GP2GP.

Catchment Area (Practice Boundary)

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

The practice has a requirement to assign an ‘Accountable GP’ to all patients on the practice list. All patients will be assigned to a GP in the practice and this will be recorded on their medical record as their ‘Usual’ or ‘Named’ GP. It is the patients right to express a preference of ‘usual/named’ GP for their medical matters, if you have a preference please get in touch and inform the medical administration team.
