Patient Information for Face to Face Appointments

We are aware that the phone lines are particularly busy at the moment and the calls we are taking do tie up the lines, especially at 8am. Please be assured that a new phone system is being installed in the very near future, which we hope will reduce the time it takes to get through.

We hope this advice is useful to you, please follow and monitor our website/Facebook page for updates.

Boughton Health Centre is also part of the Chester East Primary Care Network (PCN)

Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.

To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

The aim of primary care networks is to build on the core of current primary care services to support patients to make informed decisions about their own health and care and connect them to a wide range of statutory and voluntary services to ensure they can access the care they need first time.

We have joined together with Heath Lane Medical Centre, Park Medical Centre & Upton Village Surgery to create Chester East PCN. In total we cover a population of approx. 37,100 people and have been working together since 2018 to build the relationships required to deliver an even better service for our patients.
