Dear Patient,

patient surveyWe would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we have set up a patient participation group so that you can have your say. We will ask the members of this participation group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We may contact you via email and keep any surveys succinct so it shouldn’t take too much of your time.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

The Patient Participation Group (PPG) are introducing a new 0n-line addition to the traditional meeting system.

It is expected to become operative in July and recruitment is taking place now for people who would like to participate with the PPG on-line. It is intended to be a 2-way operation, patients will be able to raise and discuss issues with the Group as they would in a conventional meeting and the PPG will be able to keep in contact with Boughton Health Centre patients.

If you are interested, either to join or consider joining please send your name and e-mail address to www.boughtonppg@gmail.com<mailto:www.boughtonppg@gmail.com>

Your contact will be acknowledged along with a summary of what the Patients group does
