Patient (EMIS) Access

Patient Access Patient Access is a secure Internet system whereby patients can perform, on-line, certain tasks usually carried out over the telephone or in person.



We also have a new service that allows patients to consult with a GP electronically, and offers alternatives to calling or coming into the surgery for common, more minor problems. Using this service you can also access self-help content and alternative options available to you (e.g. pharmacy, symptom checkers). Patient Access Login Page

If you register to use Patient Access without getting your details from the practice, you will have limited access to the available services. Please note that you must visit the practice to confirm your registration within 1 month otherwise your application will be rejected and you will have to re-register for the service.

As we can only offer Patient Access to patients aged 16 and over we can allow you to book an appointment in your name for your child. You MUST change the name to your childs name at the reception desk when you check in. PLEASE NOTE - 1 appointment is for 1 patient. A clinician can only see 1 patient per appointment

To confirm your registration, 2 forms of documentation must be provided as evidence of identity and one of these must contain a photo.

Acceptable documents include: 

  • Birth Certificate
  • Marriage certificate
  • Medical card
  • Driving license
  • Passport
  • Local authority rent card
  • Paid utility bill
  • Bank/building society card statement
  • National insurance number card
  • Payslip
  • Letter from benefits Agency/Benefits book/signing on card
  • Papers from the home office
  • P45

If you feel you have any of the following symptoms you can now book an appointment online via patient access with a Nurse Practitioner. On booking please state the problem for which you are attending. If you leave this blank we will need to contact you to ensure that it is appropriate for a Nurse Practitioner. If you are unsure if the symptoms you have are appropriate for a Nurse Practitioner then please contact the surgery on 01244 325421.

Patient Access – Conditions that can be seen by a Nurse Practitioner:

  • Abdominal Pain

  • Bites & Stings

  • Back Pain

  • Chest Infection

  • Children with fever (over the age of 1)

  • Constipation

  • Coughs, Colds, Sore Throats

  • Diarrhoea & Vomiting

  • Emergency Contraception

  • Eye/Ear infections

  • Headache for more than 12 hours

  • Limb pain (no injury)

  • Limb injury (more than 48 hours ago)

  • Minor head injury (not knocked out, not vomiting and you are not taking Warfarin)

  • Neck Pain

  • Nosebleeds

  • Rashes

  • Removal of superficial splinters

  • Scratches, cuts and bruises

  • Small burns (not caused by electricity or chemicals)

  • Thrush/Cystitis symptoms

  • Urine Infections

  • Vaginal bleeding/discharge
